After you create a survey, you can assign it to specific parishes/facilities.

   

 To assign a survey
  1. On the Parishes & Facilities tab, click Surveys.
  2. Select a survey from the survey list, and select Make Available.
  3. Click Assign this Survey to Parishes/Facilities.
  4. Select the check box next to each parish/facility you want to assign the survey to. Click Select a Group to open the Selection window.
  5. Click Save > Close.

   

After a parish/facility submits a survey response, you can view and edit the response.

  

 To view and edit a survey response
  1. On the Parishes & Facilities tab, click Surveys.
  2. Select a survey from the survey list.
  3. Click View/Edit a Parish/Fac. Responses to this Survey.
  4. Select the parish/facility you want to view responses for, and click View/Edit Survey Response.

 
  

Additional Field Information

Date Last Opened
The date that the parish/facility last opened the survey in PDS Office.

Date Finished
The date that the parish/facility finished the survey in PDS Office.

Submitted
The date that the parish/facility submitted the survey through PDS Office to the diocese.

Locked
After a parish/facility submits a survey, it's automatically locked. Once locked, the parish/facility can't change any more information on the survey unless the diocese unlocks it. Also, when the diocese initially sends the survey, selecting the Locked check box prevents a parish/facility from submitting a survey.
